A person’s way of life shapes his or her face – this not only affects facial expressions and mannerisms, but also the skin on his or her face. In addition to environmental influences, excessive sunbathing and frequent solarium visits, the consumption of alcohol and smoking as well as our diet leave their mark on our faces. Over time, skin and connective tissue inevitably lose their elasticity as a result of natural ageing processes, and as a result; unevenness and wrinkles, volume shifts or so-called hanging cheeks develop.

These natural developments are perceived by many as flaws – they want to do something to regain smooth facial skin and thus look younger and more vital. Aesthetic surgery has therefore developed a number of new, optimal treatment methods to achieve lasting volume effects and to visibly improve the quality of the skin.

Especially successful is the so-called lipofilling (also called lipotransfer or own fat treatment), in which the wrinkles are injected with the body’s own fat and thus smoothed. This makes the face appear firmer and more voluminous and gives it a more youthful and beautiful appearance. Since lipofilling uses the body’s own substances, this method is extremely low-risk and very well tolerated. Doctors and patients therefore increasingly prefer lipofilling to Botox or hyaluronic acid injections.

At which points is a treatment with autologous fat possible?

In principle, almost every type of facial wrinkle can be treated with lipofilling.
However, very good results can also be achieved with lipofilling or when compensating for pronounced volume loss in the face.

Which basic types of facial wrinkles are known?

We distinguish between dynamic, static and actinic wrinkles.

Dynamic wrinkles are caused by facial muscle movements and emotions and are also known as mimic wrinkles.
Static wrinkles are permanently visible wrinkles that are mainly caused by skin aging. They have nothing to do with the facial expressions of the face.
Actinic wrinkles are wrinkles that are the result of frequent sunbathing and solarium visits.

What types of wrinkles are known?

Below you will find some examples of known wrinkle types.

  • Marionette wrinkles: stretch from the corners of the mouth down to the chin and often give the affected person the expression of being in a bad mood or sad.
  • Nasolabial folds: also known as nasolabial furrows, extending on both sides from the upper end of the nostrils to the corners of the mouth.
  • Tear groove: marks the lower border of the lower eyelid, it is located at the lower bony edge of the eye socket.
  • Crow’s feet: these are wrinkles radiating outwards from the outer corners of the eyes.
  • Furrows: are vertical depressions between the eyebrows.
  • Wrinkles on the chin: these wrinkles are often caused by overstretching and overexertion of the chin muscles.
  • Wrinkles on the neck: these wrinkles are a sign of age, they often result from the loss of elasticity and resistance of the skin.

Further fields of application

What is the procedure for the injection of wrinkles with autologous fat?

In order to inject wrinkles with your own fat, your body’s own fat must first be obtained. To obtain fat, the surgeon first removes fatty tissue from another region of the body where there are sufficient fat pads. Hips, abdomen and thighs are suitable for fat extraction. This method of fat extraction is also known as liposuction and has been successfully used for a long time. For this purpose, the site selected for the fat extraction is locally anaesthetised so that the procedure is painless. The surgeon will only remove fat from areas where the removal of fat can improve the contour and where no adverse effects can occur.

Afterwards, the fat removed is processed. The fat tissue is then prepared for implantation in the face. In concrete terms, this means that it is filtered intensively in order to remove unnecessary tissue components. After it has been prepared in this way, it is filled into a syringe which is then used for injection.

Subsequently, the own fat is implanted. Similar to Botox or hyaluronic acid, the patient’s own fat is now gently injected with the finest injection needles and develops its effect after only a short time. The fact that our body does not perceive our own fat as a foreign body makes it the ideal filler for smoothing and cushioning wrinkles in the face. The procedure takes place under local anaesthesia or in twilight sleep and does not require an inpatient stay. You can go home immediately after the procedure and resume your daily routine after just two to three days. Reddening or small bruises, which can occur as a result of injury to tiny blood vessels during treatment, are no cause for concern and disappear after just a few days.

When should I not have a wrinkle injection performed?

If you are pregnant, if you have been diagnosed with hepatitis, HIV or other infectious diseases or if you have a blood clotting disorder, you are unfortunately not eligible for wrinkle treatment with your own fat or hyaluronic acid.

After treatment

After the treatment, please make sure that the regions injected with your own fat are not exposed to any mechanical stress or irritation. If a larger amount of fat has been implanted, cooling the corresponding areas helps to support regeneration. Taking painkillers should not be necessary after lipofilling.

What are the risks of wrinkle injections?

As with any surgical procedure, the treatment of wrinkles with autologous fat or hyaluronic acid is associated with certain residual risks. In extremely rare cases, wound infections, sensory disturbances, lipogranulomas (benign tumours) or cyst formation can occur after successful treatment.

What are the pros and cons of using your own fat to treat wrinkles?

First and foremost, the extraordinarily good tolerance of the autologous fat treatment for the body should be mentioned here. The body’s own tissue cannot trigger any allergies and the danger of a rejection reaction is minimal. The use of the body’s own substances also makes lipofilling a comparatively inexpensive and quick procedure that requires no preservatives or other foreign materials. In this way, the development of an infection with unknown pathogens can also be ruled out.

Nevertheless, you should also consider the disadvantages: Even the removal of one’s own fat is a surgical procedure, albeit a very small one, during which small blood vessels are affected and temporary bruises can occur. If the storage and processing of the autologous fat is not carried out carefully, the fat cells can die and are no longer usable.
